Do you accept my insurance?
Transform Physical Therapy and Wellness works for you, not your insurance company. By providing out of network services, we can work together to treat the root of dysfunction without authorization limits, copays, or double-booking patients. We can provide a superbill (itemized document) upon request, which you can submit to your insurance company for reimbursement. You are still able to use your FSA or HSA as payment.
What is wellness?
We are uniquely qualified to help support your fitness goals beyond recovering from a surgery or injury. Wellness visits could include strengthening and training progressions, injury prevention training, stretching, or promotion of general wellbeing.
Do I need a referral from my doctor?
We are always happy to partner with your other healthcare providers, however a referral is not required. Michigan allows direct-access for treatment by a physical therapist, which means we may begin working together without a referral. A treatment plan will be sent to your doctor following an initial evaluation. Wellness visits never require a referral.
What can I expect during my first session?
After scheduling your first appointment, you will have the opportunity to complete intake paperwork and begin to share your story and goals. Your first visit will include a detailed history and comprehensive exam. After we discuss findings, we can make a treatment plan together that feels comfortable to you.
What does it cost?
The cost for a 1-hour visit is $140. The cost of each session includes the convenience of therapy in your home (or wherever it is convenient for you), individualized care from someone who cares about your whole story, and a detailed action plan to work on outside of sessions. A superbill is available upon request, which allows you to submit for reimbursement through your insurance company. You are still able to use your FSA or HSA as payment, regardless of insurance reimbursement.
